Student Survey Form

Below is an automated survey.  If you are one of many students who don't like asking questions in class, I have developed this page to give you the opportunity to ask questions or offer comments anonymously.  There are a standard set of questions below, but you only have to respond to the questions that address your concern.  Just fill in the blanks and then click on the "submit" button below.  The computer will send me your comments in the form of an email, but I will not know who you are.  I will post replies to the survey on my announcement page.

1.  Explain what subject discussed in class, within the last week, you understand the most.

2.  What subject discussed in class, within the last week, do you  understand the least?  What part confuses you?

3.  What do you think could be done to improve class discussions?

4.  Do you like the sections of the class text book you've read in the last week? Do you understand what the author is      saying or does he confuse you more?

 5.  How well does your instructor explain subject matter to the  class?  Any suggestions about how to improve how information discussed or approached during class?

6. If you don't see a question that deals with the subject you wanted to address, just type your concerns in the box below.  You can look underneath the "How to Contact Me" links on my faculty website for my reply to your email by clicking on the "Survey Replies" link.  You  need to give me about 24 hours to post my replies unless it is the weekend.
