BASIC LONG-TERM DISABILITY

Administered by the Standard Insurance Company.  After a 90-day waiting period due to an extended disability, the plan pays 62.5 percent of base salary not to exceed $800 per month, less any other group disability benefits.  Permanent full-time employees working 30 or more hours per week are automatically covered if enrolled in either the State Health or HMO health plans.  

SUPPLEMENTAL LONG-TERM DISABILITY

Employees have an option to elect a 90-day or 180-day waiting period.  Monthly benefit equals 65 percent of the employee’s monthly salary up to a maximum benefit of $6,500 per month.  The monthly benefit is reduced by benefits received from annual and sick leave, Social Security, Worker’s Compensation, other group disability and state retirement benefits.

The guaranteed minimum benefit of $100 will be paid each month.  The premium is determined by the employee’s age, salary, and plan elected.
