COURSE CONTRACT AND INFORMATION SHEET  FALL 2008

Please note that I will be making significant changes to the testing procedures due to gas prices, and new accreditation guide lines. We will also be using the fourth edition of the same book. You can still use the third edition and I will provide you with the conversion differences. I am planning to have all your tests available on-line except the final exam in MyMathLab to keep you from having to drive to the Testing Center.

Sorry if this adversely affects anyone but I think it is best for the majority.

It is important for you to complete and submit the following form starting August 18, 2008 and ending August 28, 2008. 

Any student who has not submitted their course contract by August 28, 2008 will be reported as a "NO SHOW".

Please read your syllabus very carefully and completely before completing and submitting your course contract.

You will not receive your course ID until I receive your Course contract.

I have thoroughly read the entire syllabus and all supporting links and agree to all the requirements of the course.  I understand that I must have access to and fluency with a computer equipped with an e-mail program, CD ROM, speakers and sound card.

I also understand that it is my responsibility to complete all tests by the test deadlines and that my grade and course status could be adversely affected.

I agree to take my final exam at the Airport Campus test site (ASC262) during the stated hours of operation and deadline that is in my    syllabus and posted on the Testing Center's web site.

I understand that no books, notes, or formula cards can be used for the on line tests and the proctored final exam. I will not discuss the tests with other classmates.

I give my permission to have my grades emailed to the email address I provide or I will provide additional information to instructor as to how I want my grades sent. (Provide by email to me your request such as regular mail, voice mail, phone, etc.)

I understand that by clicking the "Submit" button at the bottom of this form, I have electronically signed the Course Contract and agreed to the policies and conditions of the course as outlined in my course syllabus.

PLEASE NOTE:  You will not be considered officially part of this class until I receive this form and you will not be allowed to take any tests. I will give you the course ID by email once I receive your contract and the course is available on line.
