MAT100 HYBRID COURSE CONTRACT  Fall 2009

It is important for you to complete and submit the following form starting August 17, 2009 and ending August 27, 2009. 

Any student who has not submitted their course contract by August 27, 2009 will be reported as a "NO SHOW".

Please read your syllabus very carefully and completely before completing and submitting your course contract.

You will not receive your course ID until I receive your Course contract.

I have thoroughly read the entire syllabus and all supporting links and agree to all the requirements of the course.  I understand that I must have access to and fluency with a computer equipped with an e-mail program, CD ROM, speakers and sound card.

I also understand that it is my responsibility to complete all tests by the test deadlines and that my grade and course status could be adversely affected.

I agree to take my final exam and midterm exam in the desiginated classroom during the assigned time.

I understand that no books, notes, or formula cards can be used for the on line tests and the proctored  exams. I will not discuss the tests with other classmates.

I understand that my grades will be available in MyMathLab online.

I understand that by clicking the "Submit" button at the bottom of this form, I have electronically signed the Course Contract and agreed to the policies and conditions of the course as outlined in my course syllabus.

PLEASE NOTE:  You will not be considered officially part of this class until I receive this form and you will not be allowed to take any tests. I will give you the course ID by email once I receive your contract and the course is available on line.
