COURSE CONTRACT AND INFORMATION SHEET

 

It is important for you to complete and submit the following form starting August 17, 2009 and ending August 24, 2009. 

Any student who has not submitted their course contract by August 24, 2009 will be reported as a "NO SHOW".

Please read your syllabus very carefully and completely before completing and submitting your course contract. You must cut and paste the contract in an email to me at gillespiee@midlandstech.edu

You will not receive your course ID until I receive your Course contract.

 

Each student must read, sign, date and email this form to me before the first day of classes. If this form is not returned to me before the first day of classes, I will consider you a no show.  If you register for a class and decide later not to take it, you must complete a drop form, even if you never attended the class. The drop form must be completed within the published refund schedule for the term if you want to receive a full or partial refund. Copy and paste into email.  Do not send it as an attachment  Insert your name, student number, and date; fill out the student information portion before emailing this to your instructor.

I also understand that it is my responsibility to complete all assignments in a timely manner and that my grade will suffer should my assignments not be completed by the deadline.

I promise that all work performed and submitted in this class will be my own.  I understand that should any section homework, test, or exam performed in this class be determined not to be of my own work that I will then be subject to disciplinary measures as outlined in the college catalog.
