Initial Request for Accommodations

It is the student’s responsibility to self-disclose as a student with a disability and to request accommodations through Counseling and Career Services. In order to ensure access to services and programs, all initial requests for accommodations need to occur in a timely manner prior to beginning the service or program or course of interest to the student.

To complete an Initial Request for Accommodations a student must complete the following process:

  1. Complete and submit Disability Services Intake Form ( PDF Version) (Word Version)
  2. Submit Documentation of Disability
  3. Complete a Initial Request for Accommodations Initial Interview

A request for accommodations is complete ONLY after the student has completed all three steps as outlined above.

Documentation Guidelines

The staff of Counseling and Career Services will consider any information completed by a qualified professional submitted by a student as supporting documentation of a student’s request for accommodations including, but not limited to, the following:

  • Formal Psychological-Educational Evaluation or Medical Evaluation/Assessment OR
  • Professional Letter (on letterhead) written by a health, education, or service provider OR
  • Documentation of Accommodations and Services from another College or University (e.g. Accommodation Letter, or History of Accommodation Letter) OR
  • Documentation of Accommodations and Services from High School (e.g. Individual Educational Plan, 504 Documentation, Summary of Performance Note, Transition Plan) OR
  • Counseling and Career Services Standard Documentation Form

Documentation completed by a qualified professional that is submitted by a student in support of the student’s request for accommodations, which includes current information regarding the student’s diagnosis or diagnoses, substantial functional limitations on a major life activity (e.g. learning, reading, writing, hearing, seeing, etc), and recommendations for accommodations is very helpful when determining reasonable accommodations, but is not required. 

Request for Accommodations Initial Interview

All students requesting accommodations and services need to complete an Initial Interview with a staff member of Counseling and Career Services. A Counseling and Career Services staff member will contact the student to schedule the Initial Interview appointment after reviewing necessary paperwork. Appointments are available on either the Airport or Beltline Campus based upon mutual student and staff availability.

Request for Accommodations Approval

All student requests for accommodations are carefully reviewed by the staff of Counseling and Career Services in a timely manner. The Counseling and Careers Services staff will make final decisions regarding accommodations. The student will be notified of the status of the student’s request for accommodations in writing. If the staff has any questions or concerns regarding a student’s request for accommodations, a staff member of Counseling and Career Services will follow up with the student. If a student would like to appeal the decision, the student may do so by completing the Counseling and Career Services Appeal Process.
