			To register with Counseling Services as a student with a disability: You will need to complete an intake packet: (Adobe Acrobat Reader Required for all forms) Disability Services Intake Form Voter Registration Declination Form Interested in registering to vote, you can find a Voter Registration form here. Placement Test (COMPASS or ASSET) Accommodations Form (if needed) Please view our accommodations. You will need to submit acceptable documentation from a professional qualified to diagnose your disability. Standardized Documentation Forms: ADD/ADHD Learning Disability Physical/Motor Disability Psychological Disability Traumatic Brain Injury Counseling Services staff will review your intake packet and documentation and will send you a letter informing you of the status of your request. You must inform Counseling Services each semester of your accommodation needs in each class for which you are registered. We ask that you make your requests at least 30 working days prior to the first day of classes.
